Connections to Care (C2C): JobsPlus

This model of the Connections to Care (C2C) program will integrate mental health support into City-sponsored workforce development programs known as JobsPlus for New York City Housing Authority (NYCHA) residents. JobsPlus sites, which are located at or near NYCHA developments, will work with mental health professionals who will train and coach JobsPlus staff to screen their clients for mental health needs, offer direct support when appropriate, and link to local health providers for further care if needed. The program aims to help clients address mental health barriers that may stand in their way of successful job placement or retention.
